JCHMT assessment

1. DOPS
   Over a 4 year cycle 6 DOPS need to be performed for each procedure with at least 3 assessors over 4 years.
   The procedures in renal medicine are:
   Renal Biopsy (including 2 transplants) Paperwork here
   Temporary dialysis line insertion Paperwork here
   Tunnelled dialysis line insertion (non-essential) Paperwork here
   Peritoneal dialysis catheter insertion (non-essential) Paperwork here
   The paperwork needs to be filled in triplicate!

2. Mini CEX
   Not being rolled out until Autumn 2006.
   Will require 4 per year with 2 assessors.

3. Multisource Feedback
   Required to do 2 in 4 years. Simple form but needs 20 assessors for each one.
   Each form is here plus covering note here (You’ll need 21 each). List of raters here (You’ll need 3). Educational supervisor collation forms are here (need 1) and feedback forms here (need 3).
4. At each RITA/ARCP you need to provide “End of Placement Feedback” – form here
